The purpose of this questionnaire is to get some sense of your situation, which will be helpful to make our work together more productive and to better support you according to your context and needs.
Please answer the following questions concisely. Some of the questions are quite personal and I include them to better understand your situation. It is not mandatory to answer everything. If you do not feel comfortable answering some of the questions or if you consider that they are not relevant to the type of consultation or program that you are applying for, just leave that section blank. This application includes three sections:
General Information. These are questions about contact information and the general context of your life. It will be useful to save time during your appointment and make better use of the time of the session.
Health information. Physical, mental and emotional health are relevant references in any personal growth project and the physical aspects are particularly important for activities in nature that could be included in your program.
Personal information. This information is optional, but in the case of consultations about personal issues, it can help you take a preliminary look at the general landscape of your life, as well as facilitating a more direct approach to some of the issues that may be important for you to address during our work together.
