This application form is for the position of Mobile Health Clinic volunteer. This position does not perform any medical or clinical tasks.

All volunteers must complete an Information Session and individual training prior to commencement.
By filling out this application form, you agree to be contacted by email with details about the Information Session.

Please note, all volunteers must be aged 19 years or older and must complete a Criminal Record Check.
There is no cost to the volunteer for the criminal record check. We will provide instructions for the Criminal Record Check process during the Information Session.
