Hybrid Learning Model Survey
Cedar Valley Catholic Schools will continue the Hybrid Learning Model for the month of December.  In this model, families can choose to have their student(s) participate in either online instruction or in-person instruction for the entire month.  Students will not be able to fluctuate between these two options during that month.  

Please complete the survey below to indicate what instruction type each student in your family will be using from November 30-December 21.  This form must be completed by Sunday, November 22, 2020.
