ON-SITE ATTENDANCE - Application Form Term 4
Families are required to complete this form if they are seeking permission for their child(ren) to attend on-site for any part of Remote Learning during Term 4.

The Victorian Government has stated that all students who CAN learn from home MUST learn from home.

Please note the school may contact your employer to verify work hours or if you are working from home, to ensure that Victorian Government guidelines are being followed.

Please complete one form for each child in your family per week. Students must be in full school uniform and bring lunch, etc as there will be no canteen facilities.

A new form must be completed for each week required.

All requests must be in by the Thursday of the week before (except for Week 1).

**Children experiencing ‘vulnerability’ include:
• in out-of-home care
• deemed vulnerable by a government agency, funded family or family violence service, and assessed as requiring education and care outside the family home
• identified by a school or early childhood service as vulnerable, (including via referral from a government agency, or funded family or family violence service, homeless or youth justice service or mental health or other health service)
• Where a parent/carer indicates that a student with a disability is vulnerable because they cannot learn from home, and/or informs the school the student is vulnerable due to family stress, the school must provide on-site supervision for that student. This may apply to students enrolled in specialist schools and students with a disability enrolled in mainstream schools. In rare circumstances an employee does not need a worker permit.
