Roosevelt Elementary School Improvement
This form will help us collect feedback from community members related to the performance of our school.  Please note that Mr. Shaughnessy will be the only person to see results by student name.  Anyone else that sees results will see summary information that will help families remain anonymous.  Having student name included will help Mr. Shaughnessy know who has completed the survey and who has not.  This is one method we are using to ensure every family has access to school-based information.
