Please fill out all required information below or you WILL NOT be able to move to the next question. Please skip any non required questions that do not pertain to you. Make sure to electronically sign and submit when you are finished or you will not be PRE-registered for the Thanksgiving Food Box and/or the Christmas Toy Project.  For the Christmas Toy Project you HAVE to attend registration IN PERSON to get assigned a shopping date and to pick up some of your smaller gifts.  In Person registration will be at People to People Ministries (454 E Bowman St. Wooster) Mon-Fri, Oct 30th - Nov 10th, 9-12 and 1-4.  If you are only signing up for the Thanksgiving Food Box and we have seen all your documents in 2023, we may be able to set up your appointment online (email may come from: specialprojects@ptpm.net) OR (a phone call from:  330-317-7515).
