This survey questionnaire is to provide Hancock Public Schools the requisite information needed in order to effectively execute our "Return to Learn" planning for when school resumes on September 8. Based on our Return to Learn plan, and the three options for educational delivery that we will be providing, we are asking that you complete the answers pertaining to your student (or to the requisite number of students in your household) by Wednesday, August 19.
*If your student(s) will not be attending Hancock Public Schools, we ask that you please list your student(s) name in the requisite sections, and answer the final two questions of this questionnaire.
