By submitting this form, you are acknowledging to arrange a video to be taken for the purposes of completing a coach evaluation as part of the Manitoba Soccer/Canada Soccer/NCCP Coaching certification pathway. This video will not be used by Manitoba Soccer for any other purposes than to facilitate a coach evaluation opportunity for coaches who meet the pre-requisites.

Once you have submitted this form, Manitoba Soccer will provide you with your next steps and further instructions on how to complete and submit your video evaluation.

Please note, to be considered for a video evaluation, you must have already completed the Making Ethical Decisions Multisport Module and online evaluation.

Each evaluation requires a fee of $100 which can be payable via the options below. Once you have submitted this request form, Manitoba Soccer will provide you with instructions on how to complete payment. Payment must be received before a video evaluation is confirmed.
