CUSTOM WORKSHOP REQUEST FORM
CSI - Communication and Leadership provides custom workshops focused on developing students' leadership and communication skills. Workshops are free of charge for audiences where a majority of the participants are UCSD registered students or university faculty/staff who work with UCSD registered students.

Guidelines
• Workshops must have a minimum of 10 participants and must be at least 1 hour long.
• Groups can request up to a maximum of 2 hours of custom workshops per quarter.
• Workshops can be a part of a recurring meeting, conference, training, or retreat, but should not be used to fulfill programming requirements.
• Custom workshops are available Monday through Friday between 9am and 4pm. If a workshop request falls outside of these hours, please know that workshops are subject to staff availability.
• Please provide at least 3 weeks (21 days) notice prior to your requested date(s) for scheduling and planning purposes.

Upon submitting this form, we will contact you within 5 to 7 business days to discuss the specific goals of your program and confirm if we are able to accommodate your request.
