If you prefer to pay by check, mail entry fee to: Salem Lions, PO Box 210, Salem, SC 29676. Make Checks payable to Salem Lions Charities and put "Registered online" in the check memo.

FORM INSTRUCTIONS

Data required is Name, Handicap, Dinner Guest (Yes/No), Phone, and Email for each player. Team entry requires data for Captain and 3 additional players. A single player only needs to enter data for theirself (in the Captain/Single player block). Each golfer's dinner is included in their fee ($120); dinner guests pay $30 for dinner only. Regardless for team entry or single player, you must move through the entire form to the "Submit" button at the bottom. No data is entered until you click the "Submit" button. Once you click the "Submit" button, the page will update and inform you of success. You will be given the option to immediately pay with PayPal or credit cart. If you prefer to pay by check, just close the browser page instead and send a check to: Salem Lions, PO Box 210, Salem, SC 29676. Make Checks payable to Salem Lions Charities and put "Registered online" in the check memo.
