Indemnity Form *
I, We/ hereby agree that my/our child(ren) attend Christ Church George Kids and/or Youth ministry and certify and state as follows:

That my/our child(ren)'s participation in the above and related activities is entirely voluntary and I/we accept all risk involved therein.

Accordingly, Christ Church George, its employees and/or volunteers (appointed by Christ Church George for the purpose of assisting with the above and/or transporting the child(ren) to and from the above and related activities), and REACH-SA as a denomination, shall not be liable for any loss, damage, injury or illness of whatsoever nature and howsoever caused to my/our child(ren). 

I/We waive and abandon any claims which may at any time arise as aforesaid, both in my/our own personal capacity/ies and in my/our capacity/ies as a parent/s/ legal guardian/s of my/our child(ren), and I/we expressly indemnify Christ Church George, its employees and/or volunteers and REACH-SA as a denomination any claim which may arise or be instituted.
