LWMB IFA Transfer Request
This form is to transfer funds from your LWMB IFA Account/Misc. Ledger to a TOR Payment or a LWMB fixed payment.  If you want to transfer IFA funds from your home school Charms account please visit your home school music department's website and fill out their form.


Complete this form and click submit.  Pay special attention to the IFA submission deadlines listed below.

Use exact dollar amounts only.  Submit 1 form for each payment and only 1 student per form.  Do not combine payments or students.  If you are using IFA funds for 2 or more payments please submit two forms.  

Need to know your LWMB IFA balance?  You can check the balance of your Misc. Ledger in LWMB Charms.
