This event is registration to attend, in person, Succot at Park East Synagogue.

You must read and mark all of the following statements before registering for services. *

I certify that I have read the registration guidelines and agree to abide by them.

I understand that this reservation is for one person only and each person wanting to attend will have to fill out the form individually.

I understand that registration does not confirm approval to attend services and that it takes 24hrs for my registration to be reviewed and approved.

I have done a self-check for Covid-19 exposure and, in the past 14 days, I have not had any Covid-19 symptoms (eg, cough, shortness of breath, fever, chills, muscle pain, sore throat or new loss of smell or taste).

I have not had close contact with, or lived with, someone who has had Covid-19 within 14 days prior to entering into the synagogue.

I agree to comply with the guidelines, including but not limited to wearing a face mask at all times while you are at the Park East Synagogue and wash/sanitize your hands or put on fresh gloves when you arrive while also agreeing to maintain social distancing.

I agree to stop attending services if I become symptomatic as described in the guidelines and I will inform Cassie VanLuvender (Park East Synagogue representative) immediately at cassie@pesyn.org.
