Camp Request Form

This form is to be used to request that a camper be grouped or not be grouped with another camper. If you are attending multiple days or weeks of camp, please submit a form for each week. Only one parent/guardian from the group needs to submit the form.

We will do our best to accommodate all requests that are received at least 7 days prior to the start of camp.
