This form and our programs are for Colorado Veterans and families, residing on the Western Slope.
One form for all healing arts, studio or computer access, workshop, transportation and art supply requests!
Use this form to apply for classes, workshops or healing arts sessions.
This form is also used for annual registration to use the studios, workshops, computers or other open studio access at the veterans art center and admittance to the studios or classes will not be permitted without annual registration.
This form will only need to be completed once per fiscal year.
Use this form to sign up for transportation requests.
Our studio classes are open to everyone and are free for veterans and immediate family members but will occasionally have a fee for others depending on supplies required. Healing arts sessions are only provided for veterans and immediate family members.
Use this form if you would like to request art supplies.
