City of Boston Homeless Set Aside Program Interest Form - HOUSEHOLDS W/ CHILDREN ONLY

This interest form must be completed by shelter staff, case manager, or housing support person. If you are a household who is not working with someone on housing search/case management, please email setaside@homestart.org, and HomeStart will assist you with identifying someone who can help you complete the form.

PLEASE READ CAREFULLY - If you are a provider working with individuals, please DO NOT COMPLETE THIS FORM. This form is for households w/children only. Individuals should have a Pathways assessment completed to access units in the Homeless Set Aside program. If you are unsure of how your client can be assessed, please reach out to the Coordinated Entry team at ceteam@homestart.org.
