2023-2024 Educational Consultation Services Request Form
Thank you for contacting the Oklahoma School for the Deaf! Please fill out the form below to request educational consultation services for each student within your public school that is categorized as HI. You must fill out this for for EACH student. Requests for an educational consultant to attend an IEP should be made at least 30 days in advance. Consultants must be able to visit your school and the student before attending and consulting on the IEP. Upon completion of the form, a consultant will reach out to you as soon as possible. Please check your Spam folder if you do not receive an email within a few days. OSD operates on a 4-day school week and therefore are not available on Fridays. Thank you!

It is the school district's responsibility to schedule meetings, write IEPs and goals.

Educational Consultants must be present at IEP meetings in order to sign off.  Any IEPs that are not signed by an Educational Consultant are considered out of compliance.

It is the school district's responsibility to confirm meeting dates and times with the Educational Consultant.
