PARENT AGREEMENT -  DISTRICT POLICY FOR INTERNET USE AND ELECTRONIC COMMUNICATION
Parent/Guardian: As the parent/guardian of this student, I acknowledge that I have read the district's policy on Student Use of the Internet and Electronic Communications. I understand that access to the internet and electronic communications is designed for educational purposes and that the school district has taken reasonable steps to block or filter material and information that is obscene, child pornography or otherwise harmful to minors, as defined by the Board. I also recognize, however, that it is impossible for the school district to prevent access to all materials or information that might be harmful or controversial and I agree not to hold the district responsible for any such materials and information accessed by my child. Further, I accept full responsibility for supervision if and when my child's internet or electronic communication use is not in a school setting.    (Direct link to district policy included above.)                                                           *
STUDENT AGREEMENT-DISTRICT POLICY FOR INTERNET USE AND ELECTRONIC COMMUNICATION.
Student: I acknowledge that I have read, understand and will abide by the district's policy on Student Use of the Internet and Electronic Communications. Should I commit any violation or in any way misuse my access to the school district's technology devices, including use of the internet and electronic communications, I understand and agree that my access privileges may be revoked and disciplinary and/or legal action may be taken. If I am 18 years or older, I hereby release the school district from all costs, claims, damages or losses resulting from my use of district technology devices, including use of the Internet and electronic communications, including but not limited to any user fees or charges incurred through the purchase of goods or services.  (Direct link to district policy included above.) *

I understand that the MCSD Student Handbook, including the Student Code of Conduct,  is available at mcsd.org.  Direct links included above. *
STUDENT ACCIDENT INSURANCE
K&K Insurance Group - www.studentinsurance-kk.com
Accidents can and do happen at school. The school district does not provide medical insurance to automatically pay for medical expenses when students are injured at school as this is the responsibility of the parent. MCSD recommends Student Accident Insurance provided through K&K Insurance Group. Their website, which contains policy information and pricing is at www.studentinsurance-kk.com. Printed brochures can also be obtained at district office or at any school building. I understand that the school district is not responsible for student injuries and know how to find information for insurance offered to cover students while at school. *
STUDENT FEES
Technology, courses, sports and activities.
I understand that my student will have fees for technology, courses, sports and/or activities that will need to be paid and that it is my responsibility to check in with the bookkeeper, Mrs. Jimenez to arrange for payment.  Mrs. Jimenez can be contacted at jenny.jimenez@mcsd.org. *
JUNIORS AND SENIORS ONLY:  Federal law requires that Montrose High School provide names, addresses, and telephone numbers of high school students to military recruiters upon request.  Typically, this request comes in September of each school year and it affects current registered juniors and seniors.  The same Federal law also has an opt-out provision that allows for the withholding of names of any students who request in writing that their directory information not be released.   (Links for information included above.)
