2021 Confirmation Application

In this application, you will find all of your requirements for the 2020-2021 Confirmation year. We ask teenagers submit this application with their parents for two reasons: (1) so that you understand and agree to what is required for Confirmation and (2) for our records. This form is due by 7/1/2020.

In answering the following questions, we ask:
1) That you answer the first question with your full name (ie: Kathryn Srsen).
2) That you mark "yes" for each following question, acknowledging that you understand that this is a required step to be confirmed.

We do not have the date of Confirmation yet, and therefore we have not scheduled the Holy Hour date. We will update this information as it comes available.

I promise to commit myself to cooperating fully with my parents, my sponsor, and my catechists/teachers as they guide me through the process of becoming a fully initiated member in full communion with the Roman Catholic Church.
