The 2022/2023 Essa Minor Hockey Association Annual General Meeting (AGM) will be held on Thursday, April 05, 2023, 6:00pm in the gym of the Essa Township Recreation Centre.

Please use this form to submit a nomination for the 2023-2024 season. Please submit a separate form for each position, if you are applying for more than one. You are required to have two members in good standing who support your nomination. You will be asked to provide their name and phone number.  

Forms are due by Friday, April 1st 2023.
