This Youth Participation Form is for all youth activities that are sponsored by St. Mary's Greek Orthodox Church (ie: Church School, youth group programs, etc.). This form consists of Parent/Guardian contact information, participant information, authorization form and a youth expectations. Please note that we will use one form for nursery through 12th grade. YOU MUST COMPLETE THIS FORM IN ONE SITTING. Once you have completed the form, you will receive an email of your completed form.

Please note in the participant sections, there is a spot for up to 5 children. If you don't have 5 children, you can click next until the authorization form.
