I am the parent/legal guardian of (hereinafter referred to as "CHILD") who is a student at CORNERSTONE TUTORIAL. I do hereby grant and authorize CORNERSTONE TUTORIAL the right to take, alter, edit, copy, copyright, exhibit, publish, distribute, post, and make use of any and all pictures or video taken of my CHILD with or without my child’s name, to be used and/or for legally promotional materials including but not limited to; newsletters, flyers, posters, brochures, advertisements, fundraising letters, illustrations, websites, social networking sites and other print and digital communications. This release extends to all media formats and markets not now known or in existence. This authorization shall continue until such time as I tender in writing notice of revocation to a CORNERSTONE TUTORIAL board member. *
