By filling out this form, you agree that you have bought, or will buy tickets for the International Club Raffle. Please do not fill out this form if you do not intend to pay. Not only does it make it harder for us to do what we do, but your tickets will be disregarded.

Payment (either online or mailed/dropped off) and this form are due by March 17th, tickets bought after this time will not be considered. Money mailed to or dropped at the main office of Averill Park High School (146 Gettle Road, Averill Park, NY 12018 ) should be in the form of cash or checks in a sealed envelop with your name, address and phone number included. Make sure your name and the words APHS International Club Raffle are on the outside. All checks should be made out to Averill Park High School International Club. As a note, we receive more profit if you drop of or mail money to the high school, as the online platform takes a cut of the proceeds. If you can, we would prefer you to drop off money to the high school or mail it in.

The tickets will be drawn, and you will be notified of any winnings on March 19th. We will be in correspondence regarding pickup of winnings.
