Scheduling for Healthy Nutrition Solutions
Thank you very much for your interest in scheduling your nutrition counseling with me.  I am looking forward to helping you learn how to eat differently and how to change your thinking about food to improve your health.

Please read and provide you name and date verifying that you have read and understand the scheduling habits of Healthy Nutrition Solutions.

If there are no scheduling or financial barriers, I would like you to commit to scheduling and attending 3-4 one hour sessions with me during which time there will be education provided along with constructive feedback on things to change with your food, exercise, sleep, and stress management.
Your first 3-4 appointments will be scheduled biweekly as long as there are no conflicts in matching up our schedules.
It is often less confusing for everyone if your appointments are on the same day of the week at the same time. For example, every other Tuesday at 3:00 pm.
PLEASE ALSO SCHEDULE YOUR FIRST APPOINTMENT 1-2 WEEKS OUT IF YOU PLAN TO USE YOUR INSURANCE BENEFITS FOR NUTRITION COUNSELING TO ALLOW ME TIME TO VERIFY THE BENEFITS.
After teaching of all topics has been completed, if you would like, you can schedule follow up appointments for accountability and/or for further education of more complex issues.  Follow up appointments will last 30-60 minutes based on the complexity of the issues being dealt with.  For example, looking at food records and blood sugar logs would take an hour while follow ups that don’t include review of these might only take 30 minutes.  We can discuss scheduling follow up appointments after you have completed the first 3-4 one-hour sessions.  
Prior to scheduling you are required to fill out the new patient forms.  There is a fairly extensive diet history form, medical history, and weight history form for you to complete.  I estimate it may take you up to 30 minutes to complete these forms.  You will not be allowed to advance to schedule your first appointment until these forms are completed as they are required for our first appointment.  
A credit card will be required to be on file for any self-pay charges, copayments or coinsurance from your insurance, or for broken appointment fees.  It is my sincere hope that I will not have to bill your credit card for any of these reasons but it is my policy to that move forward with our sessions I will need a credit card on file.
