On Saturday July 11

Youth will meet at Union Church by 6PM before heading out. The walk itself will be in Wan Chai and will last around 30 minutes, followed by a reflection period. We will return back to the church by 9pm for dismissal.

Please note that there will be a *maximum of 10 spots* for the kindness walk so sign up fast!

I, the undersigned, will be participating in a short Kindness Walk June 26

I recognize that there are risks involved in participating in the mission trip/retreat and hereby assume all risk of injury, harm, damage, or death in connection with my participation in it. I understand and agree that neither Union Church nor its Trustees, Deacons, CoM, directors, employees and volunteers may be held liable in any way for any injury, harm, damage, or death that may occur to me as a result of my participation in this mission trip/retreat and hereby release Union Church, its Trustees, Deacons, CoM, directors, employees and volunteers from any injury, harm, damage or death, which may occur while I am participating in the mission trip/retreat.

I authorize Union Church through its Trustees, Deacons, CoM, directors, employees, volunteers to render or obtain such emergency medical care or treatment for me as may be necessary should any injury, harm or accident occur to me while participating in the event.

I understand and acknowledge that Union Church does not provide health or medical insurance in connection with the event and I agree that I will be financially responsible for any bills incurred as a result of medical treatment, including emergency medical treatment and/or transportation to a medical facility, in connection with my participation in the event
