CMTA EVENT WAIVER

CMTA Event Liability Release Waiver Form

I do hereby release both Carrollton Music Teachers Association and Round Grove United Church and forever discharge and agree to hold harmless, the church, staff, and Association members from any and all liability, claims or demands for personal injury, sickness or death (including COVID-19), as well as property damage and expenses of any nature whatsoever which may be incurred while said participant is participating in a Carrollton Music Teachers Association event or CMTA member recital. This release includes the adult signing and the minor child(ren) listed below and will remain in effect until revoked in writing or until the teacher or student is no longer attending CMTA events. This waiver is required for participation in CMTA events at Round Grove United Church.
