To aid transition to Sir Bernard Lovell Academy please complete this Data Collection Form as fully as possible. Completion date - no later than 14th May

You should have the following information readily available before you begin.

1. Contact details for at least 2 other adults - in case of emergency.

2. GP surgery address and telephone number.

3. Details of any medical conditions and any medication that might need to be taken in school.

You will be asked to provide your consent for a range of permissions, all of which have been covered on-line in our welcome information. Please ensure you have explored this information fully before you begin.
