This waiver & declaration must be completed by the tutor prior to visiting a client or student for in-person tutoring.

The novel coronavirus, COVID-19 has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is known to spread mainly by contact from person to person. Consequently, local, provincial and federal governmental authorities recommend various measures and prohibit a variety of behaviors, in order to reduce the spread of the virus.

I commit to comply with the requirements and recommendations of Ontario, National, Provincial and local Public health and other governmental authorities

By signing this document,
1) I acknowledge the highly contagious nature of COVID-19 and I voluntarily assume the risk that I could be exposed or infected by COVID-19 by attending in person tutoring activities.   I understand that interacting in-person involves risks.  There are risks associated with participation in the tutoring and academic support programs offered by Totally Tutors Inc., such as illness arising from disease or infection (e.g. COVID-19 pandemic or any other virus or bacteria or other pathogens), and that such disease and infection can lead to long-term or chronic side-effects or even death.

2) I also acknowledge that children can pass on disease and infection to other people, and that elderly family members and those with other medical conditions (diabetes, obesity) and the elderly are at particular risk. There are risks associated with my family or household members (e.g. spouse/partner, siblings, relatives and child/children) being exposed indirectly via the Totally Tutors program, e.g. after I have visited a child in a home to provide tutoring and return home. I hereby acknowledge these risks.

3) I declare that I nor anyone in my household, have experienced cold or flu-like symptoms in the last 14 days (including fever, cough, sore throat, respiratory illness, difficulty breathing).

4) If I experience, or if anyone in my household experiences any cold or flu-like symptoms after submitting this declaration, I will not attend any activities, programs or services on behalf of Totally Tutors Inc. until at least 14 days have passed since those symptoms were last experienced.  

5) I have not travelled to or had a lay-over in any country outside Canada, or in any Province outside of Ontario, in the past 14 days. If I travel, or if anyone in my household travels, outside the Province of Ontario after submitting this declaration, I will not attend any of Totally Tutors Inc activities, programs or services until at least 14 days have passed since the date of return.  
 
6) I agree to the requirements and recommendations of National, Provincial and local Public health and other governmental authorities and to those special safety regulations put in place by as it pertains the Covid-19 Coronavirus and to adopt all necessary measures to those effects.

7) I further irrevocably release and quitclaim Totally Tutors and its directors, officers, employees, contractors and tutors of liability from and against all injuries, claims (regardless of the cause of action), demands, actions, proceedings, causes of action, awards or costs (herein collectively a “Claim”) that may arise out of my participation in, or interaction with, the programs of Totally Tutors (e.g. by providing in-home tutoring), INCLUDING WITHOUT LIMITATION CLAIMS BASED ON THE NEGLIGENCE OF TOTALLY TUTORS AND ITS DIRECTORS, OFFICERS, EMPLOYEES, CONTRACTORS AND TUTORS.  In the event that any Claim is brought against TOTALLY TUTORS or its directors, officers, employees, contractors or tutors, by me, or by any of my family then I agree to defend TOTALLY TUTORS and its directors, officers, employees, contractors and tutors from all such claims, INCLUDING WITHOUT LIMITATION CLAIMS BASED ON THE NEGLIGENCE OF TOTALLY TUTORS AND ITS DIRECTORS, OFFICERS, EMPLOYEES AND TUTORS. I hereby irrevocably indemnify and hold harmless TOTALLY TUTORS and its directors, officers, employees, contractors and tutors of liability from and against all Claims arising from or by my spouse or children arising from my work with Totally Tutors.
