As Islamic Foundation of Toronto works with the different levels of government to slowly bring back the Masjid to full service, we are required to collect some data which may help health officials with contact tracing in case of any possible cases. As such, to visit the Masjid and its premises, all of the congregants are required to fill out this form.

Please note: This form does NOT guarantee attendance for any specific prayers. Congregants will be allowed for salaahs on a physical first-come first serve basis per salaah. Proof of form completion may be required for admittance.

I agree to maintain social distancing, of at least 2 metres, wear a mask at all times, bring my prayer mats, and come prepared with wudhu, to the masjid. The masjid may not open washroom and wudhu facilities. I will also bring a bag for my shoes and place them in the bag during my visit to the masjid. I will pray all additional Sunnah, Nafl, and optional prayers at home. I will also follow the directives as given by IFT managment, staff, and volunteers to ensure the safety of myself and others. I will also not hold Islamic Foundation of Toronto (including its management, staff, and volunteers) responsible for any reason due to my attendance at the masjid, and am coming entirely at my own risk.
