Every parent/guardian having custody/charge of any child, age 5-17 on/before August 1 of that school year, shall enroll and send the child to a public or private school or provide home school. (Act 570 of 1999)

NOTE: Parent or Guardian must answer all the following questions as accurately as possible. If you do not have information that we need for enrollment, it may be necessary to delay enrollment until such information can be obtained. State law requires questions pertaining to expulsion and residence be answered truthfully under penalty of fine.
