SSD Meal Order Form for May 2020
All Information Provided Will Be Kept Confidential!
For each day, please indicate the number of children (18 or under) that will need meals. All children under the age of 18, school age or not, are eligible to receive meals.  Meals include a breakfast and a lunch, no option of one or the other. Please order meals for your children for the entire time period. If there is a day that you do not need a meal, please choose 0 from the drop down menu.

Pick up days are Mondays, Wednesdays and Thursdays (except the week of Memorial Day, see below). Now when you choose meals for Monday, you will get meals (breakfast and lunch) for both Tuesday and Wednesday. There will not be a choice of getting only one meal if you pick up on Monday or Thursday. If you pick up on Wednesday, you will get one meal (breakfast and lunch) for Thursday. If you pick up on Thursday, you will get meals (breakfast and lunch) for both Friday and Monday.

At this time, we are still hoping to provide meals through our Summer Lunch Program.  We are working out the details and will probably have meal pick up sites for selected weeks throughout the summer.  We will share more information once things are determined.
