I agree to pay on the specified times, all tuition fees, and dues to JIC School. *
In the case of absence, withdrawal or removal from the school or any of its services, I am still obliged to pay all applicable fees up until the current month. I agree if I am paying by 12-month plan and I withdraw my child before the school term ends, I am obliged to pay the difference. In case of withdrawing from school written 30 day notice required. *
I agree, once the student is granted admission to JIC, none of the paid fees is refundable. All cheques must be dated the 1st of each month and should be 12 postdated cheques. The late payment charge for the month will be $25 per child. The NSF charge for cheque or pre-authorize payment will be $45.00 *
I further understand that: If the tuition fee is outstanding, the student will not be eligible to resume classes until such time as the outstanding amount has been cleared. I also understand that the tuition fee is subject to change any time. *
Grade 3 & 6 EQAO fee is $55.00 per child. Parents are responsible for the fee. *
I understand that supervision is available from 8:35 am and ends 3:15 pm; I will pick and drop kids on time. JIC staff is not responsible before 8:35 am and after3:15pm. The safety of your child is first and foremost, parents should come in and pick up kids after school, for safety reasons students cannot go outside without adult supervision. *
I understand that it is mandatory that my child comes to school in clean and proper uniform. *
Pictures or video of the Students may be taken on outings, activities and events in the school. These pictures will only be used by the school, published in the school newsletter, and/or posted on the school’s website. *
I grant permission for a photo/image/video that includes this student to be published on the school and school website and promotion of the school *
I grant permission for my child to participate in all school activities. I grant permission for my child to leave the school premises under the supervision of a staff member for neighborhood walks and to participate in school trips. I also grant permission to use private transportation for school trips. *
I hereby grant permission for my child to be included in academic assessments and competitions that the school chooses to participate in. *
I hereby grant permission for my child to use the Internet for academic purposes. *
I understand that JIC does not have the resources or facilities to accommodate special needs students, gifted children and children with learning and physical disabilities. *
I agree to assume all costs, responsibilities, liabilities and risks in connection with the child’s enrollment as a student of JIC School including without limitation, the child's use of play and sports equipment and participation in school activities. *
I further release, remise, and discharge JIC School and Jamia Islamia Canada,its administrators, directors, officers and teachers and their respective heirs, executors, successors, and assigns, of and from all claims, demands, damages, actions or causes of action arising or to arise by reason of the child’s participation in the activities and programs of JIC School as aforesaid, and from all claims and demands whatsoever in law or in equity which I, my heirs, executors, administrators, successors or assigns can or shall or may have for or by reason of the child’s participation in the activities and programs of JIC School. *
I have read, understood, and agree to comply and follow the policies and procedures of JIC School. *
I acknowledge that I have read and understood the description of JIC School and have had all of my questions answered to my satisfaction and that I am aware of the programs which my child will be participating in as a student of JIC School. *
