This form should ONLY be used for changes to Coach(es) and/or Team Manager(s) on a WDDOA Official Rosters. Prior to completing the form below, Registrars MUST:
1. Verify that the Coach and/or Team Manager has completed ALL compliance training (SafeSport, CDC Concussion Training, and successful Background Check)
2. ADD or REMOVE the Coach and/or Team Manager in GotSport in the WDDOA 2023-24 Registration Event
3. Be sure that the Coach and/or Team Manager has a Proper ID Photo uploaded into their GotSport account
Adults submitted on this form will be officially added or removed from the team's roster as requested.
Requests will be processed in the order received. Failure to 100% comply with Items 1, 2, and 3 above will result in a delay in processing.
