This form must be submitted in a timely manner.  A timely manner is defined as within 30 minutes of the end of the round in which the infraction occurred.

After a complainant submits this form, the Tournament Director will contact the individual and conduct an informal discussion concerning the grievance(s) in this form.  After the informal discussion, the Tournament Director shall investigate the allegations in this form.  After the investigation is complete, the Tournament Director will provide the complainant with a conclusion of the investigatory findings and any applicable resolutions.

The information disclosed is confidential.  Decisions of the Tournament Director are final and may not be appealed.
