COMPLETION OF THIS FORM IS REQUIRED FOR ENTRY AT JAMIA ISLAMIA CANADA

Please note:
JIC will be operating on a First Come, First serve basis.
Prayers will be held in increments of 90 PERSONS MAX at a time.

All registrants must read and accept the terms and conditions below:

I AGREE to provide proof of this registration/confirmation upon entry.
I AGREE I will not enter the premises without a prayer mat, masks & gloves.
I AGREE that if I require a chair, I will bring my own as they will not be provided.
I AGREE to pray all sunnah, nafl, and optional prayers at home.
I AGREE to maintain social distancing at all times.
I AGREE there will be no social gatherings, handshaking, and hugging in the property.
I AGREE JIC has the right to refuse entry for who do not comply
I AGREE JIC has the right to view Photo Identification for age restrictions
I AGREE that entry will be based on a first-come, first-serve basis

I UNDERSTAND that access to Washrooms and Wudu areas will be closed
I UNDERSTAND that the Women prayer area will is temporarily closed
I UNDERSTAND that NO access will be granted to children under the age of 10
I UNDERSTAND that NO access will be granted to elders with pre-existing conditions.

I AGREE that I will not hold JAMIA ISLAMIA CANADA LTD. (including its management, volunteers, and staff) responsible for any reason, I am coming at my own risk.

JIC WILL NOT TOLERATE ANY ALTERCATIONS WITH ITS STAFF & VOLUNTEERS. OUR VOLUNTEERS WORK HARD TO ENSURE SAFETY AND SECURITY OF THE PREMISES.
