Complete this form and email a visual of the proposed installation to 2fix@pdx.edu; this can include drawings, photos, etc. Include description, overall size, and measurements of how it will fit in the proposed location.

- Requests must be received at least 21 days prior to the planned installation date
- Non-PSU Students, Faculty, and Staff should email 2fix@pdx.edu if they wish to place an installation on campus
