(3) We are aware we represent the the NGO sector and we agree with a full participation of our representative during the duration of the whole fair. (4) The person filling in this form is an official representative of our organization and his/her obligation is to inform about this declaration his/her colleagues that will participate at the fair.  (5) The Forum 2000 Foundation as the NGO Market organizer reserves the right to pause or close down the process of registration to the event.

By filling in the data on this form I declare that I agree with the processing of personal data and I hereby grant to this organization NADACE FORUM 2000, registered office in Prague 1-Hrad, postal code 11908, ID No .: 65992768, registered in the Foundation Register kept by the Municipal Court in Prague, Section N, Insert 280 , e-mail: secretariat@forum2000.cz (hereinafter referred to as the "Administrator"), consent to the processing of my personal data in the registration form above and for an indefinite period of time. Consent to the processing of personal data is defined by Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / ES (hereinafter "GDPR"). Personal data may be stored and processed for conferences and future communications between the data controller and the data subject (processing of personal data, sending information about Administrator activities, sending invitations to other events organized, co-organized, or supported by the Administrator, sending information about other Administrator activities as is a newsletter and registration for events organized by the Administrator). Personal data may be provided to the following third parties (recipients). Physical persons cooperating with the Administrator (in particular project coordinators and other relevant natural persons involved in the organization of Administrator actions). Co-organizers, sponsors, or event partners who organize with the event manager (e.g., guest lists for event input, data needed to evaluate grants for an event organized by the Administrator). The data subject claims to have been properly instructed by the Administrator about the processing and protection of personal data that the above personal data is accurate and true and that the Administrators are provided on a voluntary basis. The data subject has the right to revoke this consent at any time. Revocation of consent may be made by the Data Entity by sending an email to: registration@forum2000.cz.
