This form is for you to nominate to represent Hockey Albury Wodonga at the Hockey NSW Under 15 State Championships.

By submitting this form and trialing, the player nominated and their parent/carer agrees to abide by the Hockey Australia Code of Conduct which is part of your Hockey ACT registration.  You are also bound by the Hockey Albury Wodonga Representative Code of Conduct which is on the HAW website.  You are also committing to be a financial member with Hockey ACT in 2022.  

For all juniors there must be a parent/carer email address and all emails will be sent to the parent.

Depending on number of nominations the Trials for the team for the NSW State Championships will be advised.

Should you make the team, there is a cost of $200.00 per player plus uniform.  

U15 Boys 17-19 June Grafton Hockey Grafton Hockey Centre, Fisher Park, Oliver Street, Grafton

U15 Girls 27-29 May Parkes Hockey McGlynn Sporting Complex, Cnr Baker and Station Street, Parkes

This form must be completed and submitted to HAW before 20th April 2022.  Any issues please contact the HAW office.

If you are unable to attend the trials please send an email to representative@haw.net.au outlining why you are unable to be there.
