I/we, the parent or guardian, represent that my child is in good health and can participate in competitive soccer. I/We hereby assume all personal and financial responsibility for my child; and release and agree to hold harmless the Michigan Jaguars FC, Total Sports Wixom, and/or their respective agents from liability for injuries sustained in any sports contest or practice held under the direction of the MI Jaguars FC. I hereby authorize the coaches and officials associated with my child’s team or the Club to secure medical treatment that might reasonably be required for injuries sustained in a sports contest or practice held under the direction of the MI Jaguars FC. I/We acknowledge that program assignments are at the discretion of the Club and coaches. I/We acknowledge that the information contained on this registration is true and understand and agree that the falsification of any information on this registration will result in the expulsion of my child from the MI Jaguars FC and associated programs. *
