I, the parent/guardian of the participant, a minor, agree that I and the participant will abide by the rules and regulations of the TCHS Cheer Birthday Party and its party hostesses. Recognizing the possibility of injury associated with physical activity and in consideration for the TCHS Cheer Birthday Party accepting the participant for its party activities, I hereby release, discharge and/or otherwise indemnify Teurlings Catholic High School, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of field and facilities utilized for the program against any claim by or on behalf of the participants attendance at the party.  I also grant permission for TCHS Cheer Birthday Party supervisors to authorize medical treatment for participant in the event of any emergency in which I cannot be contacted.
