New clubs, please complete the following three steps:

1) Complete this form

2) Email your club member list & officer list to the TMOM Membership VP (membership@tmom.info).
    Please use Excel, a template for both lists can be found in the membership renewal section of the website at
     tmom.info/membership-renewal

3) Submit dues by one of two methods:
     PayPal (https://www.paypal.com/cgi-bin/webscr?cmd=_s-xclick&hosted_button_id=DZ2HLQH2KGJML)
      or
     Mail a check to TMOM Treasurer - Missy Khan, 18011 Stoney Glade Court, Houston, TX 77095

Application form, membership & officer lists and payment must all be received to complete the membership process.
