1.) I would like a funeral mass to be held at the Motherhouse: If "yes," please send all your wishes for your funeral mass to the Motherhouse Service Coordinator. If "no," please send your wishes for a simple graveside ritual to the Motherhouse Service Coordinator. *

3.) If you will not be cremated, please choose one of the following. I am most interested in being buried in:

A shroud without a casket -- Please indicate if you have a personal preference for the kind of shroud. For example: a cotton sheet, a special quilt, etc. Those who don't indicate a preference will be buried in a simple shroud made of cotton or line fabric. All shrouds must be completely composed of natural, biodegradable materials (no polyester, spandex, etc.)

6.) The cost for Sisters of Loretto is taken care by the congregation. For all other persons, including co-members, the cost are on a sliding scale as follows:

Grave (Interment) Fee: $500-$5000 This guarantees you a space in the cemetery and covers all ongoing maintenance of that space.

Burial Fee: $400-$1000 This includes the physical logistics of the burial process: digging the grave, transporting the body from the parking area to the gravesite, and burying the body.

Cremains Interment and Burial Fee: $200-$500 This guarantees a space in the cemetery, includes the physical logistics of the burial process, and covers all ongoing maintenance of that space.

Once your form has been received, you will be sent details regarding payment.

By signing this form, I signify that I have received a copy of the Nature Preserve Cemetery Regulations and want a natural burial in the Loretto Motherhouse Nature Preserve Cemetery.
