Homecoming 2021 Guest Form
Only ONE GUEST is permitted for each La Salle student who attends the dance. This form must be completed BEFORE the event and no later than September 22. Falsifying information disqualifies a guest from attending.

The guest listed on this registration form agrees to the following:
1) She will submit to drug/alcohol screening, including the use of a portable breathalyzer, upon the request of school authorities.
2) She will NOT sell, purchase, possess or show evidence of having consumed intoxicants.
3) She will comply with all school rules, regulations and reasonable requests from school authorities.
4) The event is open only to La Salle High School students and one invited guest with approved guest form.
5) No inappropriate contact or gestures permitted school grounds.

All rules and regulations also apply to the parking lot areas and automobiles. Testing positive for the use of intoxicants and/or failing to comply with all rules and regulations will result in removal from the event, suspension and/or expulsion from school and/or legal action.

Admittance to Homecoming is dependent on completion and submission of this form by September 22, 2021. If this form is not complete, the La Salle High School student’s guest will not be permitted to attend.
