OFHS Intent to Participate Form 24-25

I would like to declare my intent to participate in the College Credit Plus program for the 24-25 school year. I understand that submitting this form does not require that I participate and I may decide not to participate without consequence.

The Intent to Participate Form is due by April 1, 2024. Students that file their intent form after April 1st will also need to file an appeal with their building principal to participate. A copy of the CCP Advising Form will be emailed to parents and students after submitting.
