Life Coaching Intake Form
I'm looking forward to supporting you in our time together. This form helps me know how to pray and prepare for our time. It also helps me make the most of our time. We can start with an Introductory Session which is a one 55 minute coaching session + free 20 min. Discovery time for $65.
Included in the Introductory Session is an extra 20 minutes for what I call "Discovery time" so I can get to know you before we dive into coaching. The intro session is a good starting point to get some insight into what has you stuck and discuss your needs and goals. You receive a 55-minute coaching session plus the 20 minute "Discovery time" and you’ll have access to any of the resources and tools used during the session.
Once I receive this form I'll send you a response with details for session and payment. Sessions are confirmed once payment is received.
