While attendance is mandatory for all rehearsals and performances, sometimes things happen. This form should be completed by the parent as soon as you are aware a student will be absent from a rehearsal or performance. Emergencies do happen. Please notify Mr. Long as soon as you are able.

Please email Mr. Long in addition to completing this form so he is aware of your absence and have your student communicate the absence with their section leader.
