My name is Justin Haegele. I work at Old Dominion University.

I am asking you to take part in a research study because I am trying to learn more about determinants of physical activity, physical activity, and quality of life among adults with visual impairments. If you agree, you will be asked to complete a survey made up of 49 questions about health-related behaviors and quality of life. The survey should take about 20 minutes to complete. All responses will be anonymous and confidential.

It is OK for you to say NO. Even if you say YES now, you are free to say NO later by not completing the questionnaire. Your decision will not affect your relationship with Old Dominion University, or otherwise cause a loss of benefits to which you might otherwise be entitled.

If you complete the survey, you will have the option to enter a drawing to win win one of two $50.00 gift cards, or one of four $25.00 gift cards.
