Seven Year old Dance Registration Form
Welcome to Michele's Dance Studio! This form is for children who are seven years old in September 2020. Please fill out one registration form for each child. Make sure to choose correctly 'in person' or 'Zoom' according to your preference. Tuition amount listed is "1st Tuition", which covers the first third of the year.

After submitting the form you will be prompted to visit PayPal.me/MichelesDance to make payment. Registration is not complete until payment is made. To calculate your payment: $30 + 1st tuition fee for each class. *THE $30 REGISTRATION FEE IS REQUIRED ONLY ONCE PER FAMILY.
