The purpose of this form is to ensure all our members have read and understood our "MASC Outdoor Land Training Guidance" and "MASC Outdoor Land Training Summary" in advance of them attending outdoor land training. Members who have not completed this form will not be allowed to take part in our outdoor land training activities.

The personal data collected in this form will be processed by MASC in order to carry out our everyday activities as a Swimming Club.
