Please review the following instructions before completing this form.

1. Please verify that you meet all eligibility requirements and have all the necessary documents by reading this page: https://oiss.yale.edu/ssn.

2. Fill out this form to request the Yale related immigration status and employment eligibility letter from OISS.

3. OISS will process your request and email you an unsigned letter, as long as you meet all eligibility requirements. Please allow 5 business days for a response.

4. Print the unsigned letter and bring it to the OISS office (421 Temple Street) during walk-in hours to be signed. Please note that you will not be able to print the letter at the OISS office, you must print it ahead of time.

5. Bring this signed letter as well as all necessary documents to the SSA office to apply for your SSN.
